SUMMARY:


The Pharmacy Department is tasked with overseeing and coordinating medication management to ensure optimal pharmaceutical care for patients across all age groups within the healthcare facility.

The Clinical Pharmacist plays a pivotal role in delivering high-quality and timely pharmaceutical services to the patients.


KEY RESPONSIBILITIES INCLUDE:
*Demonstrating commitment to the organization's mission, vision, and core values.

*Maintaining and utilizing patient medication profiles; assessing drug therapy for effectiveness, potential interactions, adverse reactions, contraindications, allergies, sensitivities, and cost considerations; practicing in a proactive and self-directed manner.

*Verifying that prescribed medications and their combinations are rational; addressing any concerns regarding drug therapy with the relevant healthcare professionals involved in patient care.

*Engaging in departmental and institutional initiatives that evaluate pharmaceutical usage in patient care.

*Promoting the use of the facility's formulary among prescribers and other healthcare professionals; managing the formulary system by consulting with prescribers when non-formulary medications are requested and recommending suitable formulary alternatives.

*Conducting discharge medication education for patients receiving medications upon discharge from the pharmacy and/or upon request.

*Performing pharmacokinetic calculations and evaluations based on the therapeutic needs of patients.

*Contributing to quality assessment and improvement initiatives within the Pharmacy Department and the healthcare facility as needed.

*Assisting in the orientation, training, and evaluation of departmental staff.

*Participating in investigational drug studies to ensure proper dispensing and maintenance of research documentation.

*Being accountable for the timely, safe, and accurate distribution of medications, utilizing professional judgment regarding medication acquisition, storage, packaging, distribution, and usage.

*Ensuring accurate transcription and evaluation of physician medication orders, preparation of patient-specific unit dose medications, sterile admixture solutions, extemporaneous compounding, controlled substance distribution, and outpatient medication preparation/dispensing.

*Maintaining the quality of pharmaceuticals within the facility by conducting inspections and removing recalled, defective, expired, or improperly stored medications.

*Supervising pharmacy technicians during their shifts to ensure completion of assigned duties; responsible for the tasks performed by pharmacy technicians.

*Adhering to safety and security protocols to foster a safe environment.

*Compounding and dispensing medication orders accurately with appropriate labeling and documentation in accordance with pharmacy policies, state, and federal regulations.

*Utilizing available reference materials to address drug information inquiries and appropriately referring questions to healthcare professionals with more specialized knowledge.

*Documenting and reporting medication variances.

*Providing accurate drug information regarding therapy indications, dosages, administration, adverse effects, interactions, IV compatibility, and formulary recommendations to physicians, nurses, and patients.

*Prioritizing workload and clinical interventions, assisting departmental and facility staff as necessary to ensure optimal patient care.

*Participating in educational and training programs within the Pharmacy Department as requested.

*Attending mandatory in-service training and departmental meetings as scheduling allows.

QUALIFICATIONS:
A degree in pharmacy (B.S., Pharm.D.) from an accredited college of pharmacy is required. Completion of a hospital pharmacy residency accredited by the American Society of Health-System Pharmacists is preferred. Pharmacist licensure in the relevant state is required.
