Job Summary

The Care Coordinator will be responsible for managing and coordinating client care through various means of support, including telephonic, chats, emails, and direct client interaction. This role involves providing information and resources, assessing client needs, crisis intervention services, and facilitating warm handoffs to internal and external programs.

Key Responsibilities
  • Provide professional support to clients via phone, chat, and email
  • Assess client needs and provide resources and referrals as needed
  • Facilitate crisis intervention services and warm handoffs to internal and external programs
  • Develop and maintain relationships with community resources and service providers
  • Participate in clinical supervision and ongoing education to enhance skills and knowledge
Requirements
  • Bachelor's Degree required; Master's Degree in social work, psychology, counseling, or related field preferred
  • Relevant licensure such as LPC, LCDC, LMSW; or 5+ years of relevant experience
  • Bilingual preferred
  • Experience with substance abuse evaluation, counseling, and case management
  • Understanding of ASAM and DSM 5 diagnostic criteria for substance use disorder and mental health disorders
