Job Description

At Advanced Home Health Services, we are seeking a skilled Registered Nurse to join our team as a Nurse Care Coordinator. In this role, you will be responsible for planning, organizing, and directing home care services to meet the needs of individuals and families within their homes and communities.

Key Responsibilities:

  • Conduct initial assessments of patients and families to determine home care needs.
  • Develop and implement individualized care plans that establish goals based on nursing diagnosis and incorporate therapeutic, preventive, and rehabilitative nursing actions.
  • Provide health care instructions to patients and families as appropriate per assessment and plan of care.
  • Communicate with physicians and community health-related persons to coordinate care plans.
  • Participate in on-call duties and ensure that arrangements for equipment and other necessary items and services are available.

Requirements:

  • Registered nurse with current licensure to practice professional nursing in the state.
  • Graduate of National League for Nursing accredited school of nursing.
  • Minimum of two years' experience in public health or home care nursing is preferred.

Skills:

  • Self-directed and able to work with minimal supervision.
  • Excellent observation, problem-solving, verbal, and written communication skills.
  • Ability to organize and prioritize workload independently.
