Job Title: Care Coordinator

Planned Parenthood of Michigan is seeking a highly skilled and compassionate Care Coordinator to join our team. As a Care Coordinator, you will play a vital role in ensuring timely and accurate follow-up for patients, working collaboratively with patients, nurses, clinicians, and community partners to provide exceptional care.

Key Responsibilities:
  • Adhere to procedures and requirements regarding notification, documentation, and follow-up of normal and abnormal results as outlined in PPGC Medical Standards and Guidelines.
  • Effectively communicate follow-up plans to patients and explain treatment plans.
  • Follow up on established plans of care for Care Paths and abnormal lab results.
  • Provide nonjudgmental communication and excellent customer service for optimal patient experience.
  • Send correspondence to patients via mail and patient portal.
  • Collaborate with licensed teammates to answer patient questions in the format preferred by the patient.
  • Serve as a resource to PPGC teammates regarding follow-up information.
  • Perform mandatory STI reporting to the states of Louisiana and Texas, as indicated.
  • Work collaboratively with the Care Coordination Team, health centers, providers, and other departments to develop and maintain a person-centered care planning process.
  • Demonstrate flexibility and openness in daily work and encourage others to stay open to change and improvement.
  • Accept and readily adapt to changing priorities, new ideas, strategies, procedures, and methods.
  • Liaise with other departments, providers, and resources to establish and maintain positive working relationships.
  • Provide confidential and quality patient care services at all times.
  • Makes and encourages positive connections with all co-workers, patients, and community.
  • Work and act independently on-site or remotely with minimal supervision.
  • Take the initiative to proactively resolve barriers, support, or escalate complex follow-up cases to licensed teammates.
  • Collaborate with Communications to share patient stories, as appropriate.
  • Participate in research duties, per protocol.
  • Comply with PPGC guidelines and approved standards.
  • Other duties as assigned by supervisor.
Qualifications:
  • At least one year of experience in the medical field, preferably in reproductive healthcare.
  • Excellent customer service and communication skills.
  • Bilingual preferred.
Education and/or Experience:

High school diploma/GED required.

Language Skills:
  • Ability to read, analyze, and interpret general business periodicals, professional journals, technical procedures, or governmental regulations.
Mathematical Skills:
  • Ability to add, subtract, multiply, and divide in all units of measure, using whole numbers, common fractions, and decimals.
Reasoning Ability:
  • Ability to solve practical problems and deal with a variety of concrete variables in situations where only limited standardization exists.
  • Ability to interpret a variety of instructions furnished in written, oral, diagram, or schedule form.
Computer Skills:

Basic computer literacy - keyboarding, e-mail, word processing, internet research and navigation, calendar, practice management system, and electronic medical records.
