Role Summary
The Patient Navigator plays a crucial role in delivering care coordination services, guiding clients through the Same Day Access or scheduled intake processes. This position demands strong interpersonal, organizational, and data management abilities. The Patient Navigator ensures that clients are linked with appropriate programs and services while actively working to eliminate referral barriers that may arise during their care journey.
Key Responsibilities

- Achieve expected referral rates for both internal and external programs; monitor outcomes for assigned CMHC & CCBHC and collaborate with providers to enhance desired results. Utilize tracking and reporting to ensure quality care consistent with agency standards.
- Develop professional relationships by partnering with clients and families, focusing on their strengths, choices, and empowerment. Conduct assessments and interactions in a trauma-informed manner, integrating cultural competency across diverse populations.
- Meet or exceed productivity standards as set by the supervisor; fulfill data collection requirements for CMHC & CCBHC, including outcome measures.
- Ensure that essential client information is communicated to service providers and support staff before and after appointments. Confirm that clients have all necessary information to proceed with their treatment.
- Foster positive relationships with all resources within and outside the agency. Participate actively in administrative and treatment staffing.
- Enter CMHC & CCBHC data into required systems within specified timelines. Maintain timely and accurate documentation reflecting services provided and client responses.
- Update client assessments and information promptly, ensuring that all team members are informed of client interactions and necessary follow-up actions.
- Conduct initial in-person intake interviews with new clients, gathering demographic, financial, insurance, and health history data. Explain financial policies and treatment consent forms, ensuring completeness and accuracy.
- Open new client charts at the initiation of mental health treatment and evaluation services.
- Deliver exceptional customer service to clients, staff, and community partners.
- Assist in coordinating treatment plan updates and targeted case management tasks.
- Collaborate effectively with all staff within the served county.
This position operates in a professional office setting, utilizing standard office equipment such as computers, phones, and photocopiers.
