As a key member of our patient care team, you will play a vital role in providing screening, follow-up, and navigation to services for mental health and substance use disorders, as well as social determinants of health.

Key Responsibilities:

  • Provide patient-centered care and support to individuals and families in need
  • Develop and implement personalized care plans to address health and wellness goals
  • Collaborate with healthcare providers and community resources to ensure seamless care coordination
  • Employ population health metrics to identify patients in need of services and develop targeted interventions

Requirements:

  • High school diploma or equivalent
  • Previous experience in social services, translation services, case management, or client/customer services
  • Bi-lingual speaker/writer preferred
  • Strong written and verbal communication skills
  • Ability to work with diverse populations and maintain confidentiality
