Key Responsibilities- Coordinate with our RN Supervisor and RNs to provide nursing and personal care services
- Develop and implement comprehensive care plans to support patients and families
- Work closely with our team, including RNs, Aides, medical directors, social workers, chaplains, and volunteers
- Valid NYS LPN License
- Minimum 1 year LPN experience
- Prior hospice/home care experience preferred, but not required
- Strong computer skills and organization and communication skills
- Thorough orientation and preceptorship program
- Supportive work environment
- Competitive pay range: $24.00-$30.10
