As a Hospice Homecare Specialist, you will be responsible for providing nursing and personal care services as part of a comprehensive plan of care, ensuring the comfort, dignity, and well-being of our patients.



About the Role

We are seeking a qualified LPN with a valid NYS license and a minimum of 1 year of experience in a similar role. Prior hospice or home care experience is preferred but not required. The ideal candidate will possess strong communication and organizational skills, be able to work effectively as part of a team, and have a passion for delivering high-quality patient care.



Responsibilities

As a Hospice Homecare Specialist, your key responsibilities will include:




  • Providing nursing and personal care services to patients in their homes, as part of a comprehensive plan of care
  • Working closely with RNs, Aides, medical directors, social workers, chaplains, complementary therapists, and volunteers to ensure seamless care delivery
  • Assessing patients' needs and developing individualized care plans to meet their unique needs
  • Collaborating with the RN Supervisor and RNs to ensure accurate and timely documentation of patient care
  • Participating in training and education programs to enhance your skills and knowledge in end-of-life care
