Job Title: RN HOSPICE Care at Home

Join our team of dedicated healthcare professionals at Trinity Health, one of the largest not-for-profit, Catholic healthcare systems in the nation. We are seeking a compassionate and skilled Registered Nurse to work as part of our Hospice Home Care Team, providing care and support to patients and families in the Columbia/Greene County area.

Job Summary:

We are looking for a highly motivated and empathetic RN to join our team. As a Hospice Home Care RN, you will work closely with an interdisciplinary team to develop and implement comprehensive care plans for patients and their families. Your primary responsibility will be to provide high-quality, patient-centered care in the comfort of their own homes.

Responsibilities:
  • Coordinate with the interdisciplinary team to develop and implement comprehensive care plans for patients and their families.
  • Provide high-quality, patient-centered care in the comfort of patients' own homes.
  • Work closely with other healthcare professionals, including Aides, LPNs, medical directors, and NPs, to ensure seamless care delivery.
  • Communicate effectively with patients, families, physicians, and coworkers to ensure optimal care outcomes.
  • Participate in ongoing education and training to stay up-to-date on the latest hospice care practices and principles.
Requirements:
  • Current license to practice as a Registered Nurse in the State of New York.
  • Associates Degree in Nursing, BSN preferred.
  • At least one year of experience as an RN, preferably in Hospice/Home Care or acute care.
  • Valid Driver's License and Reliable vehicle.
  • Excellent communication and interpersonal skills.
