LPN Hospice Homecare- Full-Time Days-Schenectady/Albany

Trinity Health is seeking a dedicated LPN to join our Hospice Home Care Team in Schenectady and Albany counties. As a full-time LPN, you will work closely with our RN Supervisor and RNs to provide nursing and personal care services to patients and families.
Key Responsibilities:- Coordinate with our RN Supervisor and RNs to provide comprehensive care to patients and families.
- Work closely with our team, including RNs, Aides, medical directors, social workers, chaplains, complementary therapists, and volunteers.
- Training provided in end-of-life care.
- Valid NYS LPN License.
- Minimum 1 year LPN experience.
- Prior hospice/home care experience preferred but not required.
- Strong computer skills.
- Organization and communication skills and ability to work as part of a team.

Pay range: $24.00-$30.10, based on experience, skills, and education.
