Job Summary:

We are seeking a compassionate and skilled LPN to join our Hospice Home Care Team in Schenectady/Albany Counties. As a full-time LPN, you will work closely with our RN Supervisor and RNs to provide nursing and personal care services to patients and families in their homes.

Key Responsibilities:

• Coordinate with our RN Supervisor and RNs to develop and implement comprehensive plans of care for patients and families.
• Provide nursing and personal care services to patients and families in their homes, including administering medications, wound care, and vital sign monitoring.
• Collaborate with our interdisciplinary team to ensure seamless care coordination and communication.
• Maintain accurate and up-to-date patient records, including progress notes and medication lists.

Requirements:

• Valid NYS LPN License
• Minimum 1 year LPN experience
• Prior hospice/home care experience preferred, but not required
• Strong computer skills and ability to work in an electronic medical record system
• Excellent communication and interpersonal skills, with ability to work effectively in a team environment
