Job Summary

We are seeking an experienced Case Manager to join our team at our City Sanctuary Facility for Families with Children in Staten Island, NY. As a Case Manager, you will play a critical role in supporting families in achieving their goals and improving their overall well-being.

Key Responsibilities
  • Maintain accurate and up-to-date records of client information and service plans.
  • Collaborate with clients to develop and implement individualized service plans.
  • Connect clients with community resources, support groups, and cultural services to promote social support and well-being.
  • Assist clients with referrals to outside agencies for legal services, education, parenting, medical, and mental health services.
  • Conduct regular unit inspections to ensure a safe and healthy environment.
Requirements
  • Bachelor's or Associate degree.
  • Fluency in languages other than English, particularly Spanish, is strongly preferred.
  • Knowledge of Outlook, Word, and Excel.
