The Residential Case Manager is responsible for providing case management and housing-related services to residential clients to facilitate their adjustment to and integration into the residence and community. Key responsibilities include ensuring the timely flow of community outreach and case management activities to meet program goals and objectives, entering assessment and progress note documentation into the program database, and conducting electronic charting activities and reporting in accordance with confidentiality regulations and hospital policies.
Key Responsibilities:- Provide case management and housing-related services to residential clients
- Ensure timely flow of community outreach and case management activities
- Enter assessment and progress note documentation into program database
- Conduct electronic charting activities and reporting
- At least one year mental health experience with target population
- Experience in case management and/or residential program required
- Documentation skills, verbal communication, written communication, people skills, and conflict resolution skills
- Minimum 60 college credits required; Associates or Bachelor degree preferred in social welfare, behavioral health, or related field
- Valid driver's license in good standing
- Proficient in Microsoft Office Suite and database navigation
