Public Health Nurse I Job Description

San Bernardino County is seeking a skilled Public Health Nurse I to join our team. As a Public Health Nurse I, you will play a vital role in providing assessments of individuals, families, and communities, and coordinating access to care.

Key Responsibilities:
  • Conduct intensive case management and develop treatment plans
  • Provide consultation on medical, dental, and behavioral health assessments
  • Act as a nurse liaison with public and private agencies
  • Conduct disease control measures and provide assessments and referrals for school district students
Requirements:
  • Valid Registered Nurse license issued by the California Board of Registered Nursing (CA BRN)
  • Valid California Public Health Nurse certification issued by the CA BRN
  • Valid Healthcare Provider level Basic Life Support (BLS) certificate
  • Experience working with pediatric populations, including enhanced care management experience, certificate, or training
Preferred Qualifications:
  • Fluency in American Sign Language, Arabic, Korean, Mandarin, Spanish, and/or Vietnamese
  • Ability to use social determinants of health in developing care plans and interventions
