About the Role: We are seeking a Patient Transition Specialist to join our team at Spaulding Hospital-Boston & Cambridge. In this role, you will work closely with healthcare professionals to facilitate smooth patient transitions to post-acute facilities. Your primary responsibility will be to conduct thorough preadmission patient evaluations to ensure that patients meet the necessary criteria for admission and are clinically suitable for transfer to their designated provider.

Responsibilities:
  • You will coordinate referrals to guarantee timely transfers of patients to SRN or PHH facilities, adhering to established timeframes and liaison processes.
  • You will obtain patient assessment information from various sources and transmit data into appropriate systems (4Next/Rehab Checklist/Epic/Allscripts/naviHealth).
  • You will foster collaborative relationships with internal and external staff to ensure customer satisfaction.
  • You will nurture referral source relationships to maintain high levels of customer satisfaction.
  • You will provide support for challenging transfers using SRN/PHH clinical and supervisory resources.

Requirements:
  • A current license in Massachusetts as an RN, PT, OT, SLP, LTAC, SNF, LCSW, LICSW, MSW, BSW, LPN, PTA, COTA, or RT is required.
  • Knowledge of IRF LTAC, SNF, and/or Home Care is preferred.
  • Excellent computer skills and working knowledge of Microsoft Office Suite, as well as the ability to learn other software applications, are essential.
  • A working knowledge of Epic is helpful.
  • Broad understanding of various insurances and government reimbursements is necessary.
  • Effective written, verbal, and electronic communications, as well as interpersonal and relationship-building skills when interacting with diverse populations, are crucial.
  • You must be skilled at problem-solving while meeting demands from all parties.

Compensation: The estimated salary for this position is $75,000 per year.
