Spaulding Rehabilitation is seeking a skilled Clinical Liaison Representative to join our team. As a key member of our admissions process, you will play a critical role in ensuring seamless transitions for patients.

About the Role:

This position involves working closely with SRN facilities and referring facilities to remove barriers and create a smooth and timely transition of care. Your primary responsibilities will include coordinating the referral process, obtaining and transmitting patient assessment information, and developing and maintaining collaborative relationships with internal and external staff.

Key Responsibilities:
  • Coordinate referral process to ensure smooth transition of patients to SRN or PHH
  • Obtain and transmit patient assessment information to appropriate systems
  • Develop and maintain collaborative relationships with internal and external staff
  • Nurture referral source relationships to ensure customer satisfaction
  • Support challenging transfers using SRN/PHH clinical and supervisory resources
Qualifications and Experience:

To be successful in this role, you will need to possess a current license in Massachusetts as an RN, PT, OT, SLP, LTAC, SNF, or LCSW, LICSW, MSW, BSW, LPN, PTA, COTA, or RT. Knowledge of IRF LTAC, SNF, and/or Home Care is preferred. Additionally, you should have excellent computer skills with Microsoft Office Suite and the ability to learn other software applications.

Salary:

The estimated annual salary for this position is $85,000 - $110,000 based on location and experience.
