About the Role

We are committed to delivering high-quality, patient-centered care. As a Clinical Care Specialist, you will play a vital role in providing specialized services to patients in various settings.

Your primary responsibility will be to provide skilled nursing care to patients in their homes or other community-based settings. You will work collaboratively with an interdisciplinary team of healthcare professionals to develop and implement individualized care plans that meet the unique needs of each patient.

To succeed in this role, you must have a strong foundation in nursing principles and practices. A minimum of one year of experience in acute care settings is required, with prior home care experience being highly preferred. Additionally, you must possess excellent communication and interpersonal skills, as well as the ability to work effectively in a fast-paced environment.

Responsibilities
  • Provide skilled nursing care to patients in their homes or other community-based settings.
  • Develop and implement individualized care plans in collaboration with an interdisciplinary team of healthcare professionals.
  • Collaborate with patients, families, and caregivers to ensure seamless transitions of care.
  • Maintain accurate and detailed records of patient interactions, treatments, and outcomes.
Requirements
  • Graduate of an approved School of Nursing.
  • Current Massachusetts license as Registered Nurse.
  • Minimum of one year previous nursing experience in acute care setting.
  • Prior home care experience preferred.
