Responsibilities:

  • Conduct thorough health assessments and create personalized care plans for each resident.
  • Administer medications and treatments according to physician orders and established protocols.
  • Monitor residents' vital signs and promptly address any changes in health status.
  • Collaborate with interdisciplinary teams to provide holistic and person-centered care.
  • Assist in the development and implementation of wellness programs and activities.
  • Educate residents and their families on health-related topics and promote preventive care.
  • Respond to medical emergencies and provide immediate and appropriate interventions.
  • Maintain accurate and up-to-date resident medical records.
  • Foster a supportive and compassionate environment for residents and their families.
  • Participate in ongoing training and professional development opportunities.

Requirements:

  • You must be a graduate or future graduate from an accredited nursing program.
  • You must hold a current state license as a Licensed Practical Nurse.
  • You must be able to communicate effectively in both verbal and written forms.
  • You must have a minimum of 1 year experience as an LPN in a long-term care setting.
  • You must be a natural problem solver and able to work effectively in a fast-paced environment.
  • You must be committed to interacting professionally and with care toward residents, families, and other staff.
