Licensed Practical Nurse

We are seeking a skilled and compassionate Licensed Practical Nurse (LPN) to join our team at Willcare, a part of LHC Group. As a Home Health LPN, you will play a vital role in providing high-quality patient care and support to individuals in their homes.
Key Responsibilities:
- Provide skilled nursing care to patients in their homes, including wound care, medication management, and vital sign monitoring.
- Develop and implement individualized care plans in collaboration with the patient, family, and healthcare team.
- Conduct thorough assessments of patients' physical and emotional needs, and make recommendations for care and services.
- Collaborate with other healthcare professionals to ensure seamless care coordination and communication.
- Document patient progress and outcomes in a timely and accurate manner.
Requirements:
- Current LPN licensure in the state of practice.
- Current Driver's License, vehicle insurance, and access to a dependable vehicle or public transportation.
- CPR Certification required.
