Catholic Community Services is seeking a highly motivated and compassionate Outreach Case Manager to join our team. In this role, you will work collaboratively with our HEN team to provide critical services to vulnerable individuals and families in need of housing and essential needs assistance.
Key Responsibilities- Conduct intakes for the HEN Program, ensuring accurate and timely assessments of client needs
- Coordinate with local Coordinated Entry providers to ensure clients are screened and placed into priority pools based on vulnerability
- Provide case management services for vulnerable clients, including transportation to housing opportunities and advocacy on their behalf
- Establish and maintain relationships with local resources, including prospective landlords and service providers
- Ensure strict compliance with all CHG guidelines and maintain accurate and complete client files
- AA degree in Social Services or related field
- At least one year of experience in direct provision of social services, preferably with homeless populations
- Excellent written and oral communication skills
- Good office computer skills, with ability to maintain complex client records and update databases
- Ability to work independently and as part of a team
