Community Navigator

The Community Navigator for the Breast Cancer Patient Navigation Program will serve as a vital link between patients, healthcare providers, and community resources. This role focuses on supporting breast cancer patients by assisting them with navigating the healthcare system, accessing resources, and overcoming barriers to care. The Community Navigator will work closely with patients, families, and medical teams to ensure timely screening and diagnosis, and follow-up care, with an emphasis on culturally competent care and addressing social determinants of health.
ESSENTIAL DUTIES AND RESPONSIBILITIES
Patient Support and Advocacy:
· Ensure patients receive breast cancer prevention, screening and diagnostic services.
· -Provide personalized support to patients throughout their healthcare journey, and throughout the screening and diagnostic process.
· Act as a liaison between patients and healthcare providers, ensuring clear communication and helping to address any concerns or questions.
· Advocate for patients’ needs within the healthcare system, helping them to navigate complex medical and insurance processes.

Resource Coordination and Referrals:
· Connect patients with appropriate community resources, including financial assistance, transportation services, support groups, and counseling.
· Familiarize oneself with databases of community resources available to breast cancer patients and their families.
· Work with social workers, case managers, and other healthcare professionals to coordinate care and referrals.
Outreach and Education:
· Conduct outreach to underserved communities to increase awareness of breast cancer, early detection, and available resources.
· Organize and participate in community events, workshops, and health fairs to educate the public about breast cancer prevention and the importance of regular screenings.
Data Management and Reporting:
· Document patient interactions, care plans, and referrals in the EMR (Athena), ensuring accurate and timely records.
· Monitor patient progress and outcomes, and report on program metrics and patient satisfaction.
· Assist with the evaluation of the program’s effectiveness and contribute to continuous improvement efforts.
Collaboration and Teamwork:
· Collaborate with healthcare providers, including and primary care physicians, to ensure coordinated care.
· Participate in multidisciplinary team meetings to discuss patient cases and develop comprehensive care plans.
· Work closely with the program manager and other navigators to share best practices and improve program services.
QUALIFICATIONS

EDUCATION AND EXPERIENCE:
· Bachelor’s degree in social work, public health, or a related field preferred.
· High school diploma or equivalent with a minimum of 2 years of experience in a healthcare, social service, or community-based setting or
LANGUAGE SKILLS
Ability to speak, read and write English. Ability to read and interpret documents such as safety rules, operating and maintenance instructions, and procedure manuals. Ability to write routine reports and correspondence. Ability to speak effectively before groups of customers or employees of organization. Bilingual skills (English/Spanish) a plus
MATHEMATICAL SKILLS
Ability to add, subtract, multiply, and divide in all units of measure, using whole numbers, common fractions, and decimals. Ability to compute rate, ratio, and percent and to draw and interpret bar graphs.
REASONING ABILITY
Ability to solve practical problems and deal with a variety of concrete variables in situations where only limited standardization exists. Ability to interpret a variety of instructions furnished in written, oral, diagram, or schedule form.
PERFORMANCE IMPROVEMENT
Ability to understand and demonstrate Aunt Martha's 4-step Performance Improvement process, utilizing the methodology Plan, Do, Study, Act (PDSA).
JOB KNOWLEDGE
Ability to use technical skills and apply knowledge necessary for this position.
QUALITY
Ability to demonstrate accuracy and thoroughness while performing the duties of this position.
SAFETY AND INFECTION CONTROL
Be knowledgeable about and follow Aunt Martha's safety policies and procedures.

Current driver’s license
Current auto insurance
Ability to work late evening hours, weekend hours, and scheduled holidays, as needed
Proficiency in Microsoft Office Suite and experience with electronic medical records (EMR) systems.
Compassionate, patient-centered approach with a commitment to supporting individuals through difficult times, with the ability to work effectively with people from diverse backgrounds.

Specific vision abilities required by this job include close vision, distance vision, color vision, peripheral vision, depth perception, and ability to adjust focus.
WORK ENVIRONMENT:
This position may involve both office work and community outreach, with occasional travel to healthcare and community sites.
While in the clinic, there may be some exposure to communicable disease, body fluids, toxic substances, medicinal preparations, radiation and other conditions common to a clinical environment. The noise level in the work environment is usually moderate.
