Key Responsibilities:

  • Act as the primary resource and point of contact for patients diagnosed with dementia or Alzheimer's, assisting them and their caregivers in navigating the healthcare landscape.
  • Prepare patients and their caregivers for the initial evaluation and assessment process.
  • Facilitate access to both clinical and non-clinical support services, including meals, transportation, and other community resources.
  • Identify medical and community resources available to patients, providing referrals to other healthcare providers as necessary.
  • Establish and maintain strong, productive relationships with community partner organizations, including home health agencies, hospice services, adult day care facilities, nursing homes, and in-home caregiver services.

Qualifications:
Minimum Requirements:

  • A minimum of 5 years of experience in a healthcare environment, focusing on the care of older adults and the dementia/Alzheimer's disease process, along with knowledge of ICD-10 diagnostic codes.
  • A valid driver's license and insured vehicle.
  • A Bachelor's Degree in Social Work or a related field, or a Registered Nurse qualification.
  • Completion of initial and annual dementia care navigator training, totaling at least 20 hours, which includes both didactic instruction and experiential training.

Preferred Qualifications:

  • Experience with Cerner EMR is strongly preferred, along with moderate to advanced experience with other electronic medical records systems.
