Position Overview

We are seeking a qualified and compassionate Geriatric Nurse Practitioner to deliver exceptional patient care in an outpatient environment. In this role, you will be responsible for diagnosing and managing various health conditions, performing examinations, ordering necessary diagnostic tests, and developing comprehensive treatment plans. Effective collaboration and communication with team members and patients are essential in this position.

Ideal Candidate Profile:

  • Strong clinical expertise

  • Valid medical practice license in Arizona
  • Board certification in the relevant specialty
  • Ability to foster positive relationships with both colleagues and patients
  • Advanced proficiency in geriatric care
Key Responsibilities
  • Utilization of EMR: Cerner
  • Focus on patient demographics: Primarily dementia patients
  • Specialization in Alzheimer's and Dementia Care
  • Management of a patient panel of approximately 250 individuals
  • Conducting both office and home visits
  • Standard work hours of 8 hours, including clinic and home visits
  • Participation in shared on-call duties
Qualifications Required
  • Current Board Certification
  • Active medical license and DEA certification
  • 2-5 years of experience in the field of geriatrics
Compensation Range
$52.79- $63.98/hour
