About the Role
As a Nurse Care Coordinator, you will be responsible for prescribing, delegating, and coordinating patient care while utilizing clinical judgment based on nursing skills acquired through formal and informal experiential knowledge and evidence-based guidelines. You will work closely with patients, families, and healthcare professionals to develop and implement effective care plans that promote optimal outcomes. Your expertise will focus on navigating payor benefits, assisting patients and families in understanding insurance plan benefits, and facilitating communication and coordination of the plan of care with healthcare teams.

Key Responsibilities
• Develop and implement comprehensive care plans tailored to individual patient needs
• Collaborate with multidisciplinary teams to ensure seamless coordination of care
• Utilize disease-specific clinical pathways to ensure effective clinical management
• Provide education and facilitates learning for patients, families, and healthcare teams
• Facilitate communication and coordination of the plan of care with healthcare teams
• Identify and address actual and potential barriers in service or treatment and work with appropriate resources across the continuum of care

Requirements
• At least 4 years of pediatric nursing, case management, care management, care coordination, utilization review, or community-based nursing experience
• BSN degree
• Registered Nurse in the State of Texas upon hire
• Accredited Case Manager (ACM) or Certified Case Manager (CCM) or Care Coordination and Transition Management (CCTM) certification
