Responsibilities
  • Coordinate care across the continuum for pediatric patients, working closely with multidisciplinary teams to ensure timely and effective transitions.
  • Develop and implement individualized care plans that meet the unique needs of each patient.
  • Collaborate with healthcare providers, nurses, social workers, and other stakeholders to promote access to care and facilitate communication.
  • Identify opportunities for process improvements and implement changes to enhance the quality of care.
Requirements

To be successful in this role, you will need:

  • A minimum of 4 years of experience in pediatric nursing, case management, or care coordination.
  • A Bachelor's degree in Nursing or a related field.
  • Current Registered Nurse licensure in the State of Texas.
  • Accredited Case Manager (ACM) or Certified Case Manager (CCM) certification preferred.
