Key Responsibilities
- Collaborate with multidisciplinary teams to develop and implement comprehensive care plans
- Utilize advanced nursing skills and knowledge to coordinate care throughout the continuum of care
- Work closely with patients, families, and caregivers to promote seamless transitions and optimal health outcomes

Requirements
- At least 4 years of pediatric nursing experience or equivalent in case management, care management, care coordination, utilization review, or community-based nursing
- Registered Nurse in the State of Texas
- Basic Life Support for Healthcare Providers certification required
- Accredited Case Manager (ACM) or Certified Case Manager (CCM) or Care Coordination and Transition Management (CCTM) preferred
