About the Role

We are seeking a highly skilled Pediatric Care Coordinator to join our team at Children's Health in Dallas, Texas. This is a full-time position that requires a Registered Nurse with experience in care management, case management, and utilization review.

Job Responsibilities
  • Coordinate patient care across the continuum, ensuring seamless transitions between healthcare settings.
  • Develop and implement individualized care plans, taking into account patient needs, preferences, and goals.
  • Collaborate with multidisciplinary teams to ensure comprehensive care coordination and efficient resource allocation.
  • Advocate for patients and families, addressing barriers to care and promoting access to resources.
Requirements
  • A minimum of 4 years of pediatric nursing experience or equivalent in care management, case management, or utilization review.
  • Active registration as a Registered Nurse in the State of Texas.
  • Accredited Case Manager (ACM) or Certified Case Manager (CCM) certification preferred.
  • Bachelor's degree in Nursing (BSN) required by December 31, 2023.
Salary and Benefits

The estimated salary range for this position is $80,000 - $110,000 per year, depending on experience.
