Key Responsibilities- Conduct comprehensive assessments to determine the care needs of patients.
- Develop, implement, and evaluate individual patient care plans in coordination with caregivers and physicians.
- Provide intermittent skilled nursing care to pediatric and adult patients at their residences.
- Supervise paraprofessional staff and licensed practical nurses, ensuring compliance with state and federal regulations.
- Maintain accurate documentation and process orders, admissions, transfers, and discharge paperwork.
- Ensure ongoing communication with all disciplines involved in patient care.
- Adhere to federal, state, and agency policies and procedures.
- Graduate from an accredited school of nursing with an associate or bachelor's degree in nursing.
- Active RN license in the state of Texas.
- At least one year of nursing experience in home health care, with pediatric experience preferred.
- Knowledge of state and federal home health agency regulations and compliance standards.
- Current CPR certification from an American Heart approved course.
- Valid driver's license, reliable transportation, and active auto insurance.
- Ability to perform physical assessments and provide care in non-wheelchair accessible homes.
- Successful completion of a background check.
