Angels of Care Pediatric Home Health is seeking a skilled Pediatric Home Health Nurse to join our team. As a Pediatric Home Health Nurse, you will provide high-quality patient care to infants, children, and adolescents in their homes, working collaboratively with our multidisciplinary team to ensure continuity of care.
- Assess patients' post-acute needs and develop individualized care plans.
- Provide direct patient care, including administration of medications, treatments, and therapies.
- Coordinate care with other healthcare professionals and family members/caregivers.
- Educate patients and families on clinical care to enhance positive outcomes.
Requirements:
- Active Texas RN or LPN license.
- Active CPR certification.
- Trach/vent experience preferred.
- Home care experience preferred.
