Job Title: Diverse Care Initiative Grant Project Director

At HCR Home Care, we are seeking a highly skilled and experienced professional to lead our Diverse Care Initiative Grant Project. The successful candidate will be responsible for managing and coordinating all aspects of the project, including grant administration, community outreach, and team management.

Key Responsibilities:
  • Oversee grant administration and management, including planning, budgeting, and tracking deliverables.
  • Develop and implement outreach strategies to Black churches and community organizations to promote home care services.
  • Lead a small team of community liaisons to assist with outreach and administrative tasks.
  • Represent HCR Home Care at conferences, health fairs, and community events to promote our services and build relationships with key stakeholders.
  • Develop and implement educational programs and materials to promote home care services and address healthcare disparities in the African American community.
Requirements:
  • Master's degree in a related field, such as healthcare administration, public health, or social work.
  • Minimum 5 years of experience in grant management, community outreach, and team leadership.
  • Excellent communication and interpersonal skills, with the ability to build relationships with diverse stakeholders.
  • Strong analytical and problem-solving skills, with the ability to manage multiple projects and priorities.
