Job Summary

The Diverse Care Project Director will oversee the management and coordination of HCR Home Care's project aimed at addressing healthcare delivery disparities among African American senior citizens.

This role will serve as the primary point of contact in the community, fostering relationships with Black religious leaders who refer individuals in need of home care services.

The Project Director will lead a small team of community liaisons, ensuring the successful execution of project goals.

Key Responsibilities

  • Grant Management: Oversee all aspects of grant administration, including planning, budgeting, tracking deliverables, and submitting reports.
  • Grant Activities: Implement the scope of work outlined in the grant project plan, developing and implementing outreach to Black churches and delivering patient referrals for Black seniors.
  • Team Leadership: Lead a small team of community liaisons to assist with outreach and administrative tasks.
  • Communication: Develop and deliver presentations to Black churches, representing HCR Home Care and promoting home care services.

Requirements

This role requires a strong understanding of home health care, exceptional customer service skills, and the ability to work within the schedules of Black churches, which may include evenings and weekends.
