Job Summary

We are seeking a highly skilled Care Coordinator Social Work LMSW to join our team at Nuvance Health. As a key member of our care coordination team, you will play a critical role in ensuring the smooth transition of patients from our hospital to their next level of care.

Key Responsibilities
  • Conduct comprehensive assessments of patients to identify their needs and develop individualized discharge plans.
  • Collaborate with the Clinical Care Coordinator to design and implement discharge plans, ensuring a safe and timely transition for patients.
  • Provide counseling and crisis intervention services to patients and their families, addressing issues such as substance abuse, financial needs, and grief counseling.
  • Assess patients for abuse and neglect, and report findings to appropriate agencies and internal departments.
  • Initiate and coordinate applications for conservatorships, working with legal counsel and courts as needed.
  • Lead patient and family conferences, providing education on advanced directives and patient rights.
  • Complete readmission and CARL tools for patients readmitted within 30 days of discharge.
  • Ensure that the Clinical Care Coordinator is informed of any changes to discharge plans or barriers to a safe and timely discharge.
Requirements

To be successful in this role, you will need:

  • A Master's Degree in Social Work (LMSW) and a minimum of three years of experience in counseling and discharge planning in an acute care hospital or equivalent.
  • Demonstrated discharge planning and implementation skills, with the ability to analyze financial and social situations and identify problems and alternative courses of action.
  • Excellent communication and interpersonal skills, with the ability to work independently and as part of a team.
  • Ability to prioritize tasks and manage multiple responsibilities in a fast-paced environment.
Working Conditions

This role requires:

  • Some manual skills and motor coordination.
  • Medium to heavy physical effort, with the ability to exert up to 35 pounds of force.
  • Generally pleasant working conditions.
