Nuvance Health is seeking a skilled Clinical Care Coordinator to join our team at Norwalk Hospital. As a key member of our Emergency Department, you will play a critical role in ensuring the efficient progression of care for our patients.

Key Responsibilities:

  • Manage the Utilization Review process for patients requiring hospitalization, ensuring the correct level of care and optimal initial care plan.
  • Collaborate with physicians, nurses, and other healthcare professionals to identify and address patient needs.
  • Develop and implement effective discharge plans, coordinating with community resources and healthcare providers as needed.
  • Monitor and analyze clinical, financial, and social situations to identify opportunities for improvement.
  • Provide education and support to patients and families on hospitalization guidelines and discharge planning options.

Requirements:

  • Associate's Degree in Nursing and a minimum of three years of job-related experience.
  • Registered Nurse license in the State of Connecticut with a minimum of one year of experience as an acute Care Coordinator.
  • Knowledge and expertise in electronic medical records, Interqual, and Milliman screening criteria.
  • Excellent communication and collaboration skills, with the ability to work effectively with physicians and other healthcare professionals.

Preferred Qualifications:

  • Certification in Case Management or willingness to obtain.
  • Bachelor's Degree in Nursing.
