Job Summary

We are seeking a highly skilled Care Coordinator Social Work Specialist to join our team at Norwalk Hospital Association. As a key member of our Care Coordination team, you will play a critical role in providing exceptional patient care and ensuring seamless transitions between healthcare settings.

Key Responsibilities
  • Conduct comprehensive assessments of patients' needs and develop personalized discharge plans in collaboration with the Clinical Care Coordinator.
  • Implement discharge plans, ensuring timely and safe transitions for patients, and coordinate with Care Coordination Assistants to confirm services and equipment.
  • Provide counseling and support to patients and families in crisis, including substance abuse counseling, financial need referrals, and grief counseling.
  • Assess patients for abuse, neglect, and domestic violence, and report to appropriate agencies and internal departments as needed.
  • Lead patient and family conferences, provide Advanced Directive resources, and facilitate post-acute hospital care selection.
  • Complete readmission and CARL tools for patients readmitted within 30 days of discharge.
  • Fulfill all compliance responsibilities related to the position.
Requirements
  • Master's Degree in Social Work (MSW) or equivalent.
  • Minimum three years of experience in counseling and discharge planning in an acute care hospital or equivalent.
  • Demonstrated discharge planning and implementation skills.
  • Ability to analyze financial and social situations, identify problems, and alternative courses of action.
  • Flexibility, resourcefulness, and creativity in problem-solving.
  • Prioritization skills and ability to act independently.
Working Conditions

This role requires medium to heavy physical effort, with the ability to exert up to 35 pounds of force. The working environment is generally pleasant, with little to no potential for occupational risk.
