Job Summary:

Sunrise Senior Living is seeking a highly skilled and compassionate Care Manager Coordinator to join our team. As a key member of our community, you will be responsible for providing exceptional care and support to our residents, while also leading and managing our care team.

Key Responsibilities:

  • Lead and manage the care team to ensure high-quality care and services are delivered to residents
  • Participate in resident evaluations and contribute to the development of resident Service Plans (SP)
  • Maintain appropriate staffing levels and create daily assignments based on resident evaluations and assigned levels of care
  • Recognize changes in resident condition and take appropriate action, including making notifications as needed
  • Access, use, and document actions in Sunrise systems, including the electronic health record (Sunrise Care Connect)
  • Participate in the community's weekly Interdisciplinary Team (IDT) meeting to review and discuss selected residents and any changes in care needs or Service Plan interventions
  • Partner with the Activities & Volunteer Coordinator (AVC) to ensure resident participation in activities
  • Facilitate care manager involvement in activities as staffing allows
  • Engage in proactive communication with families on a monthly basis to provide updates on resident condition and life in the community
  • Develop resident schedules for bathing, laundry, and housekeeping services
  • Conduct monthly Lead Care Manager Meetings

Quality Assurance and Regulatory Compliance:

Follow all federal, state, and local laws and regulations, as well as Sunrise standards, to ensure regulatory compliance and quality care delivery

Strive for excellence in quality care and service delivery, participating in QAPI meetings and Performance Improvement Plan committees as required

Maintain a thorough knowledge of current and evolving state laws and regulations, policies, and procedures related to resident care and community operations

Partner with leadership to ensure community compliance with OSHA requirements

Assist in the leadership and promotion of all Sunrise policies and programs related to resident care, quality, safety, and risk management

Financial Management:

Explain and discuss the value of Sunrise's products and services to residents, families, team members, and targeted referral sources

Assist the Executive Director in completing the annual community budget, as needed

Understand and manage the department budget, including labor and other expenses, and discuss its impact on the overall community budget

Review monthly financial information and implement plans of action for deficiencies

Process and submit monthly expenses and budget data timely, per Sunrise policies and internal business controls

Coordinate with the community team to achieve maximum staff economies and cross-training when appropriate

Understand the internal costs associated with Sunrise resident care programs, such as care levels, medication programs, and continence products

Training, Leadership, and Team Member Development:

Perform overall management of the department, including recruiting, hiring, training, coaching, and performance improvement

Provide training to care manager staff on their job responsibilities and resident activities of daily living

Partner in the delivery of Sunrise University, onboarding, and annual training, as needed

Review timekeeping practices and payroll reports to ensure all hours worked are recorded and appropriately paid

Conduct timely performance appraisals with meaningful conversations

Hold team members accountable and correct actions when necessary, documenting conversations and actions taken

Attend regular community meetings, including Stand Up, Cross Over, Department Head, Town Hall, QAPI, IDT, and others as required by the Executive Director

Keep abreast of professional developments in the field by reading, attending conferences, and training

Maintain compliance in assigned required training, as applicable to this role, to ensure Sunrise standards are always met
