Job Summary

We are seeking a highly motivated and organized Discharge Planner and Community Resource Liaison to join our team at WLCFS Wisconsin Lutheran Children. As a key member of our team, you will be responsible for providing exceptional case management services to our clients, while building and maintaining strong relationships with community organizations and stakeholders.

Key Responsibilities
  • Provide case management services to clients, including discharge planning, attendance tracking, and transportation arrangements.
  • Build and maintain relationships with community organizations, identifying new opportunities for referral-generating relationships and developing new ideas for services.
  • Communicate STRONG's positive treatment outcomes and impact with key stakeholders in the community, including identifying new opportunities for referral-generating relationships and developing new ideas for services.
  • Facilitate effective case management, including maintaining regular attendance spreadsheets and facilitating tours, meetings, written and audio/visual communication.
  • Collaborate with the team to ensure the quality continuum of care, including ensuring the quality of care and services provided to clients.
Requirements
  • Bachelor's degree in Social Work, Psychology, Counseling, or a related clinical field.
  • Minimum of two (2) years' experience in a clinical setting, including case management.
  • Preferred two (2) years' experience in sales, outreach, or public relations in a healthcare setting or related community relations experience.
  • Competency in use of Microsoft office, including Word, Outlook, Excel, and Salesforce CRM.
  • Aligned with STRONG mission and vision and passionate about working in a community mental health setting.
  • Previous experience with Utilization and Review.
  • Works independently and with a team in a shared workspace.
  • Communicates respectfully and professionally with supervisors, coworkers, clients, and other professionals.
  • Strong problem-solving skills.
  • Excellent customer service skills.
  • Valid driver's license.
  • A high degree of responsibility to maintain confidentiality in all matters of the division.
  • Responsible, energetic, honest, courteous, reliable person, with a tactful demeanor in dealing with public and other Agency staff.
