Job Summary:

As a Clinical Pharmacist, you will serve as the clinical pharmacy liaison to the site or system service line, demonstrating expertise in clinical practice, including appropriate drug therapy for patients and prevention/resolution of drug-related problems. You will provide clinical support to others for more complex cases, applying in-depth knowledge in a specialty area. You will solve complex problems and use specialty expertise as a resource to assist others, ensuring all team members are aware of unique patient situations.

Key Responsibilities:

• Optimize medication-related continuity of care at admission, transfer, and discharge for the service line.
• Lead medication safety advancements and performance improvement activities for pharmacy services in the specialty service line.
• Routinely participate in specialty work groups, including councils, committees, and ad hoc teams.
• Advance change in the service line to ensure clinically appropriate, cost-effective medication therapies.
• Facilitate change implementation within the service line, including physicians, nurses, and pharmacists.
• Design drug distribution processes to meet patient care needs in the service line.
• Communicate patient-specific dispensing needs to all team members in the functional/specialty area.
• Lead pharmacy department training in the service line, orienting/training new pharmacy team members, and precepting students and residents, coordinating resident/student rotations.
• Provide input to development and improvements in training programs, precepting resident projects, and accountable for team member development in the service area.
• Provide front-line staffing in the specialty area as called upon, maintaining advanced skills in practice areas, and serving as an exemplary role model for all pharmacists in the service area.

Requirements:

• Pharmacist license issued by the state in which the team member practices.
• Bachelor's Degree in Pharmacy.
• Typically requires 5 years of experience in specialty residency training or pharmacy.
• Specialty certification required (for external hires, certification must be obtained within one year of hire).
• Demonstrated decision-making, team building, customer service, and conflict resolution skills.
• Maintains advanced skills in areas of practice, demonstrating a commitment to continuing professional growth and advancement of the pharmacy profession.
• Strong computer skills, excellent communication and organizational skills, and demonstrated presentation, facilitation, and training development skills.

Physical Requirements and Working Conditions:

• Must be able to stand for long periods of time.
• Will occasionally lift up to 35 lbs. without assistance.
• Must have functional speech, vision, and hearing.
• Will be exposed to chemical hazards.
• Operates all equipment necessary to perform the job.
• Exposed to normal office environment.
