Position Overview

The Director of Case Management & Discharge Planning plays a pivotal role in overseeing the development, execution, and sustainability of Case Management and Discharge Planning protocols within the organization.

  • Leadership Responsibilities: This position reports directly to the Chief Medical Officer and supervises a dedicated team of approximately 25 full-time employees. The Director will engage with patients, families, healthcare providers, and external partners to ensure robust advocacy for patients and their families.
  • Operational Oversight: The Director is accountable for managing inpatient denial processes, departmental financial planning, completion of Patient Review Instrument (PRI) assessments, and the Alternate Level of Care (ALC) procedures. Additionally, the Director will facilitate quarterly evaluations of the PEPPER report.
  • Quality Assurance: This leader will ensure the management of inpatient, outpatient, and observation cases, guaranteeing that patients and their families receive consistent, high-quality care that aligns with organizational, state, and federal standards.
  • Innovative Solutions: The Director must exhibit a high degree of creativity, teamwork, and analytical skills to provide insights and recommendations for ongoing initiatives aimed at reducing patient length of stay, resource allocation for uninsured patients, and overall organizational growth.
  • Community Engagement: Seeking a proactive leader who will actively participate in departmental, organizational, and community initiatives, including task forces and compliance committees, to enhance community resource availability.
  • Regulatory Knowledge: Candidates should possess a comprehensive understanding of accreditation standards, regulatory requirements, payer systems, and Family Healthcare Decision Act initiatives to ensure compliance with local, state, and federal laws.
  • Collaboration and Communication: The Director will promote effective collaboration and communication, fostering positive relationships with physicians, home care providers, social services, primary care offices, and hospice organizations.
Qualifications
  • A Bachelor’s degree is mandatory.
  • An active New York State Registered Nurse (RN) license is essential.
  • Relevant healthcare experience is required.
