Transforming Care Delivery with UnitedHealth Group

We are seeking a highly skilled Per Diem Advanced Practice Clinician to join our Senior Community Care team at UnitedHealth Group. As a key member of our interdisciplinary team, you will play a vital role in delivering high-quality, patient-centered care to our most vulnerable populations.

Primary Responsibilities:

  • Conduct comprehensive assessments to identify the unique needs of each patient
  • Develop and implement personalized care plans to address the medical, social, and functional needs of patients
  • Manage patients through acute medical situations or exacerbations of chronic conditions, including testing, diagnostics, and interventions
  • Ensure accurate and complete documentation, including ICD-10 conditions
  • Communicate and collaborate with the interdisciplinary care team to ensure seamless care coordination
  • Conduct advanced illness and advanced care planning conversations to support patients and their families
  • Provide patients and caregivers with counseling and education to promote self-management and independence

Requirements:

  • Active and unrestricted license in the state of Florida or ability to obtain by start date
  • Certified Nurse Practitioner or Physician Assistant through a national board
  • Current, active DEA licensure/prescriptive authority or ability to obtain post-hire, per state regulations (unless prohibited in state of practice)
  • 1+ years of hands-on post-grad experience (long-term care setting preferred)
  • Ability to gain a collaborative practice agreement, if applicable in your state
  • Availability to work 24 hours per month, with expectations that 16 of the 24 hours/month could be during off-hours (after 5 pm, on weekends, and/or holidays) not to exceed hours in a calendar year
  • Ability to complete physical requirements of the job, including lifting a 30-pound bag in and out of the car, navigating stairs and a variety of dwelling conditions, and sit, stand, and kneel to perform physical assessment
  • Access to reliable transportation

Preferred Qualifications:

  • Experience in meeting the medical needs of patients with complex behavioral, social, and/or functional skills
  • Understanding of Geriatrics and Chronic Illness
  • Understanding of Advanced Illness and end-of-life discussions
  • Proficient computer skills, including the ability to document medical information with written and electronic medical records
