Housing Case Manager Job Description

Job Summary: We are seeking a highly motivated and compassionate Housing Case Manager to join our team at Evergreen Treatment Services. As a Housing Case Manager, you will play a critical role in helping individuals achieve stability and improved quality of life through outreach, engagement, and ongoing relationships.

Key Responsibilities:

  • Provide outreach and housing-focused case management services in a culture-informed and trauma-informed manner.
  • Assist clients in obtaining housing and maintaining housing stability.
  • Develop rapport and a trusting relationship with clients through respectful outreach and client-centered engagement practices.
  • Assist clients with establishing and maintaining primary medical care and addressing chronic health conditions.
  • Assess nature and severity of clients' substance use issues and match clinical interventions to each individual's stage of change.
  • Address clients' needs for mental health and substance use disorder treatment services and facilitate engagement in appropriate behavioral health supportive services.
  • Advocate for the client with a wide variety of other service providers.
  • Maintain a professional relationship with clients' landlords, while advocating for the clients' housing stability.
  • Document activities according to agency, contract, and state requirements.

Requirements:

  • Undergraduate degree with coursework in the social service field preferred.
  • A minimum of two years' experience in work related to social work or outreach programs.
  • Valid Driver's License and acceptable driving record required.
  • Experience providing services from a harm reduction perspective highly desirable.
