The Clinical Pharmacist is responsible for delivering comprehensive pharmaceutical services through integrated medication management, clinical support, and educational initiatives.

Key Responsibilities:

  • Medication Management:
    • Ensures accurate verification and assessment of medication orders using the Cerner system.
    • Evaluates patient profiles to confirm the appropriateness of prescribed medications, considering factors such as dosage, potential side effects, and cost-effectiveness.
    • Oversees the accuracy of all dispensed medications.
    • Adheres to investigational drug protocols and maintains compliance with sterile compounding regulations.
    • Manages narcotic dispensing and resolves any discrepancies.
    • Ensures adherence to pharmacy policies and relevant laws regarding drug dispensing.
    • Proposes enhancements to pharmacy operations and workflows.
  • Team Leadership:
    • Actively supervises pharmacy technicians and interns, providing mentorship and training.
    • Fosters collaborative relationships with medical and nursing staff, ensuring clear communication regarding drug availability and regulations.
  • Independent Problem Solving:
    • Effectively addresses challenges with minimal supervision, including drug shortages and discrepancies.
    • Participates in the development and revision of policies and procedures related to medication management.
  • Clinical Expertise:
    • Completes thorough patient monitoring and makes therapeutic recommendations based on established guidelines.
    • Engages in continuous quality improvement initiatives and adheres to clinical programs.
  • Educational Contributions:
    • Trains and mentors pharmacy students and residents.
    • Provides educational sessions for medical staff and nursing personnel.
  • Additional Duties:
    • Performs other related responsibilities as needed.
Qualifications:
  • Doctorate in Pharmacy (Pharm.D) from an accredited institution.
  • Minimum of 2 years of experience in a hospital or ambulatory care setting, or completion of a residency program.
  • Strong organizational and time management skills.
  • Excellent analytical and problem-solving abilities.
  • Proven ability to collaborate effectively with healthcare professionals.
  • Ability to work independently and as part of a team.
  • Commitment to high standards of patient care and service.
Licenses/Certifications:
  • Current Registered Pharmacist license with the California State Board of Pharmacy.
  • Basic Life Support (BLS) certification from an accredited provider.
  • Completion of Fire Life Safety Training as required.
