Key Responsibilities:

  • Oversee the recruitment and management of the Social Services team, serving as a vital resource.
  • Establish departmental objectives, ensuring clear communication and regular meetings with the Social Services team.
  • Ensure compliance with policies and procedures within the SNF; recommend updates and revisions as necessary.
  • Develop individualized Social Services care plans for residents, identifying goals and strategies while participating in interdisciplinary care planning sessions.
  • Engage in quality improvement processes within the SNF.
  • Provide counseling to residents and their families, facilitating family meetings as needed.
  • Conduct admissions, psychosocial assessments, and advanced directives, ensuring proper documentation in medical records.
  • Manage referrals for health services and support the discharge planning process for the department.
  • Complete all necessary documentation for medical records and generate monthly departmental reports for the SNF Administrator.
Qualifications:
  • Master's degree in Social Work; LMSW preferred but not required.
  • A minimum of 3-4 years of experience as a Licensed Social Worker in a Nursing or Rehabilitation Facility.
  • At least 1 year of experience in a Senior or Director position.
  • Strong understanding of available social services and community resources.
  • Exceptional communication skills are essential.
