Key Responsibilities:
- Oversee the hiring and management of the Social Services team, serving as a vital resource.
- Establish departmental objectives, ensuring clear communication and regular meetings with the Social Services team.
- Ensure compliance with facility policies and procedures, recommending updates and revisions as necessary.
- Develop individualized Social Services care plans for residents, identifying goals and strategies while participating in interdisciplinary care planning sessions.
- Engage in quality assurance processes within the facility.
- Provide counseling to residents and their families, facilitating family meetings as needed.
- Conduct admissions, psychosocial assessments, and advanced directives, ensuring proper documentation in medical records.
- Manage referrals for health services and oversee the discharge planning process for the department.
- Complete all required documentation for medical records and generate monthly departmental reports for the facility administrator.
- Master's degree in Social Work; LMSW preferred but not required.
- A minimum of 3-4 years of experience as a Licensed Social Worker in a Nursing or Rehabilitation Facility.
- At least 1 year of experience in a senior or director-level position.
- Strong knowledge of available social services and community resources.
- Exceptional communication skills are essential.
