We are seeking a highly skilled and compassionate Patient Services Coordinator Home Health to join our team at Enhabit Home Health & Hospice. About the Role

The Patient Services Coordinator Home Health will be responsible for scheduling patients to branch field clinicians, communicating with field staff, patients, physicians, referral sources, caregivers, and other service providers to maintain proper care coordination and continuity of care. This role requires strong organizational and communication skills, as well as the ability to work effectively in a fast-paced environment.

Responsibilities
  • Schedule patients to branch field clinicians
  • Communicate with field staff, patients, physicians, referral sources, caregivers, and other service providers
  • Manage the on-call notebook and hospitalization logs to enhance communication among stakeholders
Qualifications
  • High school diploma or equivalent required
  • Licensed practical or vocational nurse in the state in which they currently practice, with at least one year of clinical experience in a healthcare setting, or at least one year of home health, hospice, or pediatric experience within the last 24 months, and a demonstrated understanding of staffing and scheduling requirements related to home care services
  • Basic demonstrated technology skills, including operation of a mobile device
