About the Role

We are seeking a highly skilled and compassionate Patient Care Coordinator to join our team at Enhabit Home Health & Hospice. As a Patient Care Coordinator, you will play a vital role in ensuring the seamless delivery of high-quality patient care in the comfort of their own homes.

Key Responsibilities
  • Schedule patients to branch field clinicians to ensure timely and efficient care delivery.
  • Communicate effectively with field staff, patients, physicians, referral sources, caregivers, and other service providers to maintain proper care coordination and continuity of care.
  • Manage the on-call notebook and hospitalization logs to enhance communication among stakeholders.
Requirements
  • High school diploma or equivalent required.
  • Licensed practical or vocational nurse in the state of practice, with at least one year of clinical experience in a healthcare setting, or at least one year of home health, hospice, or pediatric experience within the last 24 months, with a demonstrated understanding of staffing and scheduling requirements related to home care services.
  • Basic demonstrated technology skills, including operation of a mobile device.
Preferred Qualifications
  • Previous experience in home health, hospice, or pediatrics.
Additional Requirements
  • Valid state driver's license.
  • Maintain automobile liability insurance as required by law.
  • Maintain dependable transportation in good working condition.
  • Ability to safely drive an automobile in all types of weather conditions.
