Job Summary: We are seeking a skilled Community Habilitation Specialist to provide respite care services to individuals with special needs. The ideal candidate will have experience working with individuals with special needs and possess excellent communication and interpersonal skills.

Responsibilities:

Assist individuals with daily living activities, such as bathing, dressing, and grooming

Provide training and supervision to help individuals develop skills and increase independence

Develop and implement individualized plans to meet the needs and goals of each individual

Maintain accurate records and reports of services provided

Requirements:

High school diploma or equivalent required; CAN/HHA experience preferred

Ability to work independently and as part of a team

Excellent communication and interpersonal skills

Ability to lift 25 pounds and perform physical tasks as needed

Working Conditions:

Work is performed in a community-based setting, with frequent travel to individuals' homes and community locations

May be required to work flexible hours, including evenings and weekends

Must be able to maintain confidentiality and handle sensitive information with discretion
