Job Summary

This is an entry-level position in a direct care setting, assigned to community residences. The incumbent will work shifts to ensure 24/7 coverage, performing a variety of tasks related to the care, nurturing, treatment, education, socialization, habilitation, recovery, safety, security, and support of adults with mental illness and substance abuse disorder.

Key Responsibilities:

  1. Conduct regular inspections of the facility to ensure the safety of consumers and residents, accountability of property, cleanliness of consumer rooms and common areas, and security of the area outside the facility.
  2. Report serious incidents, allegations, incidents, or sensitive situations to supervisors, completing incident reports in accordance with Institute for Community Living policy.
  3. Explain services, recreational activities, and programs available to consumers and residents, instructing them in daily living skills, socialization, and conflict resolution.
  4. Execute emergency plans under the direction of the assigned supervisor, adhering to program policy on securing the safety and well-being of consumers, staff, and visitors.
  5. Assist and instruct individuals in personal hygiene, grooming, nutrition, and daily living, ensuring sufficient and appropriate attire.
  6. Review the staff communication log, making entries concerning security checks, crisis incidents, emergency situations, incident reports, or other essential information.
  7. Observe the physical environment, performing housekeeping tasks as necessary to maintain a safe, clean, comfortable, and healing environment.
  8. Ensure food provided or selected is suitable for those on a special diet.
  9. Accompany consumers or residents on scheduled or emergency visits to medical treatment facilities, social agencies, social activities, government offices, or other locations associated with treatment or assistance.
  10. Comply with attendance rules, reporting to work as scheduled and/or required on a regular basis, with availability and ability to work beyond the normal schedule as needed.
  11. Attend regular team and staff meetings, and supervision.
  12. Comply with all required in-service training and staff development activities.
  13. May provide monitoring of medications and make appropriate medication administration form entries, assisting consumers/residents on a monitored medication regime.
  14. Assist clinical staff in accounting for residents/consumers, filing missing person reports as necessary.
  15. Observe behavior and responses to treatment and rehabilitation programs, support services, and medication, reporting observations as instructed.
  16. May lead skills development or activity groups with individuals and families.
  17. May assist and supervise individuals in meal preparation, laundry, and light housekeeping tasks.
  18. Other job-related duties as assigned.

Requirements:

  • High school graduate or GED, plus one year of related human services experience. Specialized training or education in human services may be substituted for experience.
  • NYS driver's license may be required for some assignments.
