Job Summary

Connected Home Health & Hospice is seeking a skilled Certified Nursing Assistant to join our team in the Greater Portland area. As a Home Health/Hospice CNA, you will provide personalized care and support to patients in their homes, promoting a healthy life/work balance.

Key Responsibilities
  • Provide personal care and treatment to patients in their homes
  • Plan and prepare meals, assist with feeding if necessary
  • Perform range of motion and simple procedures as an extensional therapy service
  • Take and record temperatures, pulse, respiration, and blood pressure as ordered
  • Assist with laundry and address patient safety needs
  • Report on patient's condition to the assigned nurse and participate in regular Interdisciplinary Group (IDG) Case Conference meetings
Requirements
  • CNA licensure in Oregon
  • 1+ years of experience preferred
  • Ability to work independently and demonstrate critical thinking
  • Current BLS certification from AHA
  • Be a licensed driver with access to a reliable vehicle
