Key Responsibilities:
  • Conduct initial and ongoing assessments of patients' physical, functional, spiritual, psycho-social, and environmental needs.
  • Partner with an interdisciplinary team to develop and implement plans of care, prioritizing patient and family needs.
  • Communicate effectively with patients, families, and healthcare professionals to ensure seamless care coordination.
  • Prepare and submit accurate documentation of patient observations, findings, and services provided.
