Care Partners, a renowned Oregon nonprofit hospice alliance, is seeking a dedicated Hospice Aide to join our team. As a Hospice Aide, you will play a vital role in providing compassionate care to patients and their families in the Portland metropolitan area.
Key Responsibilities- Provide personalized care to patients, adhering to individualized plans of care and visit frequencies.
- Assist patients with bathing, oral care, skin care, and hair care, as well as ambulation and positioning for comfort.
- Report changes in patient condition and needs to ensure optimal care.
- Maximize patient comfort, safety, and health, aligning with hospice philosophy and organizational policies.
- Maintain accurate and up-to-date patient records, reflecting plans, actions, goals, and problems.
- A minimum of one year of prior CNA, Hospice Aide, or equivalent work experience.
- A current certificate of practice as a Certified Nursing Assistant (CNA) in the state of Oregon.
- Basic Life Support CPR card for Health Care Provider and First Aid certification.
- A valid driver's license and acceptable driving record, with access to a reliable automobile for patient visits.
- Excellent communication and documentation skills, with the ability to work effectively with patients and families in terminal care.
- Proficiency in electronic tools, including email, internet, keyboarding, and medical software for accurate charting.
- Reliability and ability to work independently and as part of a team, delivering assistance within scope of practice.
- Ability to lift and move patients, with prolonged standing and walking.
- Bi-lingual, Spanish-speaking ability is a plus.
